Home Care

Home care includes nursing services, personal services, nutritional services, and up-keep services. Home care services are provided by our licensed, class A home care agency. These services are also available off-campus in the greater Northfield community.

Northfield ParkView Home Care offers a full range of:

  • Personal health care services
  • Medication management
  • Companion services
  • Assistive Remote Monitoring Care

A registered nurse will determine the levels of care required.  Individuals will have a full assessment when health services are requested. Unless there is a major change in condition, the levels of care will remain consistent for an entire month.  If or when a major change in condition occurs, an assessment will be conducted and care levels adjusted accordingly.

Initial Assessment

This service provides an RN to visit the home care client to assess the individual’s specific care needs.  Offer recommendations for care.  Set up services requested.  Contact physician for information to assist with cares.

60 Day Nurse Assessment

This service provides a Licensed Nurse/Registered Nurse to visit every 60 days, at a minimum, more often if needed.  Nurse will take vitals, review health status and plan of care.  Make adjustments as needed.  A fee will occur for each nurse visit.

Assistive Remote Monitoring Care

This personal response system provides a pendant for alerts to a designated person for summoning assistance for emergencies.  This service also includes a motion or contact sensor.  This can be set to alert a designated person of lack of motion or movement. For more information, click here.
